“Non-medical support and practical assistance”

man_holdinghandTransitions is a volunteer-based program designed to provide non-medical support and practical assistance. Transitions coordinators meet with patients and families to assess needs, then coordinate volunteer services and community resource information.

Transitions volunteers are specially trained to provide assistance and are experienced and caring people. They offer support, friendship and respite for patients and their families. They help with errands, food preparation, friendship and more.

The Transitions coordinators serve as guides, helping identify needs, providing emotional support for decision making and pointing patients and families toward valuable community services.

Transitions is available to anyone who has been diagnosed with an advanced illness and has a prognosis of one year or less. To speak with the Transitions coordinator and learn more about the program, call 941-552-5922.

There are no charges for Transitions services — the program is fully funded through Tidewell Hospice. Volunteers and staff visit clients at home, in the hospital or at an extended-care facility.
